Nelson Care Center in Alexandria, Minnesota has been approved by the Minnesota Department of Health for a moratorium exception project—a competitive state process that authorizes major renovations for nursing facilities. This exciting project marks a $15.3 million investment in the future of skilled nursing care for older adults in Douglas County.

A once-in-a-generation investment in skilled nursing care

Over the next 18 months, beginning in early 2026, Nelson Care Center will undergo a phased renovation of its short-term rehab, Maples, and Pines households. These updates are designed to create a more welcoming, modern, and supportive home for residents and team members alike.

Renovations will feature:

  • State-of-the-art resident rooms with private bathrooms and walk-in showers.
  • Private climate control for comfort in every room.
  • Inviting dining and activity spaces designed to encourage connection and engagement.
  • Upgraded nursing stations and care areas that support efficient workflows for teams.

These improvements will bring Nelson Care Center in line with today’s CMS, ADA, and infection prevention standards, ensuring safety, accessibility, and high-quality care for all residents.

What is a moratorium exception project?

Minnesota has a moratorium, or restriction, on new nursing home construction. However, the state allows for moratorium exception projects—highly competitive applications that let facilities move forward with renovations, replacements, or upgrades if they demonstrate need and community benefit.

Nelson Care Center’s approval through this process signals strong confidence in our ability to continue serving the people of Alexandria and Douglas County with excellence.

Looking ahead

The project is expected to begin in early 2026 and take approximately 18 months to complete, with work done in phases to minimize disruption to residents and families. Throughout the process, Nelson Care Center remains committed to delivering compassionate, high-quality care for older adults.
