Advance care planning

Our team provides guidance and support to help individuals make informed decisions and feel more confident about the future. This may include conversations about values, goals, and health care preferences, choosing a decision-maker, completing or updating health-care directive documents, and sharing wishes with family members and care teams.

Who this support is for: 

  • Adults of any age planning ahead
  • Individuals living with chronic or serious illness or those who are experiencing cognitive changes
  • Anyone wanting clarity and peace of mind about options for future care

Advance care planning can be revisited at any time as health needs or life circumstances change.

Dementia care and caregiver support

Our team works alongside individuals living with dementia and their caregivers to manage changing needs, connect with resources, and improve quality of life. This may include education and guidance, care coordination and planning, support and coaching for caregivers, help connecting with respite care or other community resources, and ongoing check-ins.

Who this support is for:

  • People living with dementia
  • Family members and caregivers
  • Individuals needing ongoing dementia-related support

Some individuals may qualify for additional support services through Medicare-covered programs. Our team can help determine eligibility.

Emotional wellness

Through PEARLS (Program to Encourage Active, Rewarding Lives), our team provides short-term, one-on-one coaching designed to help older adults improve mood, strengthen coping skills, and reconnect with meaningful activities and routines.

PEARLS uses evidence-based problem-solving and goal-setting strategies to support emotional well-being in a practical, supportive way. Rather than ongoing therapy, the program focuses on building skills and routines that help individuals feel more connected, confident, and engaged in daily life.

  • Older adults experiencing low mood or depression
  • Individuals feeling isolated or disconnected
  • Older adults looking to improve emotional well-being
  • Caregivers needing support focused on emotional wellness

Care management

Care management provides long-term, personalized support to help individuals navigate complex health, social, and daily living needs while maintaining stability and independence.

Our team works closely with individuals, caregivers, health-care providers, and community services to coordinate care and ensure support stays aligned with changing needs over time. This may include coordinating care across providers and services, support with managing chronic conditions, assistance in navigating health-care and community systems, collaborative care planning, and ongoing follow-up and reassessment.

  • Individuals with multiple or ongoing needs
  • People living with chronic or complex health conditions
  • Individuals experiencing social, medical, or functional challeneges
  • Older adults needing long-term coordinated support

Resource navigation

Resource navigation provides short-term assistance connecting you to helpful local services and community resources. Our team helps identify available programs, makes referrals, and ensures connections are successful. This might include transportation, food support, housing or utility assistance, community programs, help completing applications, and follow-up support.

  • Older adults looking for community support
  • Families navigating resources

Caregiver education and support

Our caregiver education and support services help caregivers feel more confident, informed, and supported throughout their caregiving journey.

We provide education, coaching, emotional support, and opportunities to connect with others in similar situations. Support may include one-on-one caregiver consultation and coaching, education about caregiving strategies and self-care, support groups and learning opportunities, and guidance navigating caregiving challenges and available resources.

  • Family caregivers
  • Unpaid caregivers supporting older adults
  • Individuals caring for someone living with dementia or chronic illness
  • Caregivers seeking education, problem-solving support, or emotional support
