Nelson Care Center in Alexandria, Minnesota has been approved by the Minnesota Department of Health for a moratorium exception project—a competitive state process that authorizes major renovations for nursing facilities. This exciting project marks a $15.3 million investment in the future of skilled nursing care for older adults in Douglas County.
Over the next 18 months, beginning in early 2026, Nelson Care Center will undergo a phased renovation of its short-term rehab, Maples, and Pines households. These updates are designed to create a more welcoming, modern, and supportive home for residents and team members alike.

Nelson Care Center has a long tradition of service, caring for nearly 300 residents in 2024 with the support of more than 326 team members. This renovation ensures that the people of Douglas County will continue to have access to modern, high-quality skilled nursing care for decades to come.

Minnesota has a moratorium, or restriction, on new nursing home construction. However, the state allows for moratorium exception projects—highly competitive applications that let facilities move forward with renovations, replacements, or upgrades if they demonstrate need and community benefit.
